3. Click on the following boxes (the form changes depending on which boxes are checked):

     -- “Internal User”
     -- "Direct Office"
     -- “letterTRAX”
     -- “Commonwealth” and/or “Lawyers”

4. Fill in your User Profile information

5. Fill in your User Notification information

6. Click on “Submit”
